Qualifications: The service coordinator position requires a bachelor’s degree in a human service field.
Essential functions: This position conducts intakes, completes initial and annual eligibility determinations, develops the Individualized Family Service Plan, links families to needed service providers, monitors services, and facilitates transition to Early Childhood Education or other services if needed.
Location: Hillside, IL
Qualifications: The Individual Service Coordinator position requires a bachelor’s degree in a human service field and one year of experience working with individuals with intellectual disabilities.
Essential functions: This position determine individuals eligible for services annually, completes the discovery tool necessary to develop the Person Centered Plan. Completes required visits to ensure health and safety of the individual, and to ensure services are being implemented.
Bilingual staff needed.
Location: Hillside, IL and Homewood, IL
Qualifications: The Individual Service Coordinator position requires a bachelor’s degree in a human service field and one year of experience working with individuals with intellectual disabilities.
Essential functions: This position completes initial eligibility determinations for services, completes the discovery tool necessary to develop the Person Centered Plan, introduces and links individuals to potential service providers, and secures the necessary funding for services through the Division of Developmental Disabilities.
